چکیده

Renal involvement is a significant complication of multicentric Castleman's disease (MCD) and various glomerular involvements have been reported. A 56-year-old Chinese woman presented with proteinuria and skin rash, with lymphadenopathy and hypergammaglobulinemia. Lymph nodes and skin biopsy proven the case was multicentric CD with plasma cell pathological pattern. The renal biopsy was performed and six glomeruli were observed and two of these showed global sclerosis. Moderate increasing of mesangial matrix with mesangial cell proliferation were seen in every glomerulus. In addition, one-segmental sclerosis accompanied by adhesion of the Bowman's capsule was revealed. Two of the glomeruli had crescents formation. Under immunofluorescence microscopy, immunofluorescence for anti-IgA, IgM, C3, C1q and FRA showed coarse and fine granular depositions along capillary walls and sparsely in the mesangium. Staining for anti-IgG was negative. Under electron microscopy revealed indiscriminate amyloidal deposits in glomerular basement membrane. The foot process of glomerular podocytes was fusion. Moderate increasing of mesangial matrix and mesangial cell proliferation were found. Subsequently, she was successfully treated with prednisone combined with cyclophosphamide therapy not only for proteinuria but also for renal function.

Successful treatment of a mesangial proliferative glomerulonephritis with interstitial nephritis associated with Castleman's disease by an anti-interleukin-6 receptor antibody (tocilizumab).

We report a case of mesangial proliferative glomerulonephritis with interstitial nephritis associated with multicentric Castleman's disease (MCD) successfully treated with an anti-interleukin-6 receptor antibody (tocilizumab). This mesangial proliferative glomerulonephritis with interstitial nephritis was resistant to methylprednisolone treatment; however, it was markedly improved with tocilizu...

A Rare Case of Castleman’s Disease of Plasma Cell Type Complicated with Membranoproliferative Glomerulonephritis

Castleman’s disease (CD) is an uncommon atypical lymphoproliferative disorder with different histological and clinical forms. Patients with the plasma cell type frequently present with systemic symptoms; including fever, weight loss, hemolytic anemia, hypoalbuminemia, hypergammaglobulinemia and increased acute-phase proteins. Renal manifestations like proteinuria or hematuria are common in Cast...

C3 mesangial proliferative glomerulonephritis initially presenting with atypical hemolytic uremic syndrome: a case report

BACKGROUND Hemolytic uremic syndrome is characterized by acute renal failure, thrombocytopenia, and Coombs-negative hemolytic anemia. In C3 mesangial proliferative glomerulonephritis, an increase in mesangial cell proliferation without thickening in the glomerular capillary wall can be seen under light microscopy, but the definitive diagnosis is made with the immunohistologic demonstration of i...

Histologic studies on the nephrotic syndrome in the elderly.

Renal histopathology in 87 patients, aged over 60, with nephrotic syndrome were studied. In 57 patients diagnosed as primary glomerular disease, membranous glomerulonephritis (MGN) was of the most common histologic type (52.6% of the cases), and mesangial proliferative glomerulonephritis (21.1%), membranoproliferative glomerulonephritis (12.3%) and minimal change (12.3%) were the other types of...

Steroid-responsive nephrotic syndrome in a patient with proliferative glomerulonephritis with monoclonal IgG deposits with pure mesangial proliferative features

A 78-year-old woman developed acute-onset nephrotic syndrome. A renal biopsy showed mild mesangial proliferative glomerulonephritis. Immunofluorescence studies revealed granular IgG3- λ deposits within the mesangial area and along the glomerular capillary walls. Electron microscopy showed mesangial and subendothelial granular electron-dense deposits. The pattern of deposition was predominantly ...
